Consent to release In order for Schmidt Associates, PC to release tax returns to a third party, like a bank or insurance company,  we have to have a signed release form.   For your convenience we have blank forms available to you to print and return to us via mail or fax. Please feel free to contact us if you have questions about how to fill out the Consent to Release forms. A brief explanation follows for each corresponding blank. Your name as it appears on your tax return; if a joint return you need to add your spouses name; for a business return the name of the business. The year  or years of the tax return(s) you would like released; The individual and the 3rd party institution's name ( Bob Smith of Money Bank); The purpose (obtaining a loan, etc.).
